Not on the Birth Certificate. What do I do?

What to do when you find that you have not been put on the birth certificate as the dad?

Contact the Department of internal affairs in the first instance and explain how this came about. They may investigate further and direct you to taking a Paternity test which will cost around $350 to $400 dollars. You may be able to avoid this by providing other evidence such as proof of relationship, visits to midwife, Anti natal classes, GP, txt messages, Attendance at birth etc. Hopefully internal affairs can then negotiate a correction to the child’s birth certificate.

In our experience this has happened to a few of our dads. The Birth Registration application does say that both parents are to complete and sign but the mother or father? Can tick one of the following:
The child is born as a result of a woman acting alone in an assisted human production……Provide evidence from a registered medical doctor etc.
One parent is any of the following

-Unknown
-Dead
-Missing
-Of unsound mind
-Unable to sign by virtue of a medical condition
The other parent is overseas….. State circumstances in a Statutory Declaration
Requiring the other parent to sign the form would cause unwarranted distress to either to either of the parents. A letter from a Barrister, Solicitor, Doctor, Police report, Statutory Declaration providing evidence
